Legal Name Change
For all relevant information regarding legal name changes, refer to the Legal Name Change Form (LNC) and Guidelines. A legal name change is only applicable when there is no change of bond ownership.
Who can request a legal name change
An individual can legally change their name by reason of marriage, adoption, divorce, annulment, separation in marriage or legal name change.
If you also wish to change your address or Direct Deposit information, please ensure that you submit the Change of Address and Direct Deposit Forms (COADD and/or COADDRP) along with your Legal Name Change form.
How to submit your request
- Complete the Legal Name Change Form (LNC)
- Once fully completed, mail the form, along with the applicable documents to the attention of Transfer & Exchange Unit in Customer Service.
